1 Where to Submit Requests for Disclosure, etc.

2 Request Form and Submission Method

Please attach the required documents to our designated request form and send them by recorded delivery, simple registered mail, or another postal method that enables confirmation of the delivery record.

3 Method for Confirming the Identity of the Requesting Person

Please submit a copy of any one of the following: (1) driver's license, (2) passport, (3) foreign resident registration certificate, or (4) health insurance card, together with a copy of a certificate of residence issued within the past three months.

4 Fee Amount and Collection Method

For disclosure of personal information and notification of purpose of use, a fee of JPY 1,000, including tax, will be charged for each request.

If special work is required for disclosure, such as masking part of image data, actual costs will be charged separately. Please consult with us in such cases. Please enclose JPY 1,000 in postage stamps together with the submitted documents.

5 Matters Required to Identify the Retained Personal Data Subject to the Request

Please state the required matters on our designated request form.

6 Response Method for Requests for Disclosure, etc.

Within one month after receiving the request form, we will respond by restricted-delivery registered mail sent to the requester's address stated on the request form, or by sending a response to the stated email address.
